Client Grievance Procedure
- If you feel your rights or dignity have been violated, or you are unsatisfied with the services provided, you are entitled to submit a grievance.
- Grievances can be filed in writing, over the phone, or in person with any staff member at the agency.
- A grievance form is available on our website, or you can request one from any staff member.
- When submitting the form, include as many details as possible—such as the date, time, location of the incident, and the names of staff involved.
- Willamette Valley Mental Health will confirm receipt of your grievance within 3 business days and will begin an investigation.
- You will receive a decision on the grievance within 30 days from the date it was filed.
- If the outcome is unsatisfactory, you have the option to appeal the decision by reaching out to the Executive Director.
- Willamette Valley Mental Health keeps records of all grievances and appeals for a minimum of three years.
